Please be aware that this equipment should be covered under your practice insurance for any damage incurred whilst in your care. You will be liable for any repair charges.<\/p>\n\n\n\n

For the service to run smoothly and to ensure the availability of loan instruments, it is vital that you have the instrument returned as soon as possible after use.<\/p>\n\n\n\n

Loan agreement terms and conditions:<\/h3>\n\n\n\n
<\/p>\n\n\n\n
\n- By signing the Loan authorisation form you agree that you have read, understood and will comply with all terms and conditions associated with the loan instrument as outlined below:<\/li>\n\n\n\n
- You must inspect this loan equipment on receipt and contact us within 24 hours if any faults are found, otherwise you will be liable for any damage later identified.<\/li>\n<\/ul>\n\n\n\n

DO NOT USE IF YOU NOTICE ANY PHYSICAL DAMAGE AND WE ADVISE EQUIPMENT IS TESTED PRIOR TO USE<\/strong><\/p>\n\n\n\n<\/p>\n\n\n\n
\n- Loan equipment should be returned within 3-days of equipment being return unless otherwise agreed by VESCO.<\/li>\n\n\n\n
- Loan charges will be invoiced on return of the instrument to VESCO; the charges are left to the discretion of VESCO as to whether you are fully charged or partially charged depending on your circumstances.<\/li>\n\n\n\n
- It is your duty to ensure the instrument is returned to us, VESCO will not automatically collect any equipment from yourselves and VESCO will not be liable for any costs incurred for delayed returns.<\/li>\n\n\n\n
- This instrument should be covered under your practice insurance, in the circumstance of an instrument being damaged whilst in your care, on return to VESCO, it will be assessed, if there are any chargeable damage to the endoscope you will be contacted to inform you that the repair will be charged to yourselves at full repair price.<\/li>\n\n\n\n
- Any items sent with a loan instrument, i.e.: Biopsy instruments, mains cables, leak testers etc., which are not returned with the loan instrument, will be charged at full catalogue price.<\/li>\n\n\n\n
- This instrument should be cleaned and disinfected before returning to VESCO, if you have not reprocessed the endoscope fully then you may be charged for ETO gas sterilisation.<\/li>\n\n\n\n
- Loans sent out against service contracts\/repairs must be returned within 3 days of receiving your equipment back or the full \u00a3145 per day charge will be invoiced. <\/li>\n\n\n\n
