The number of clinics equipped with a CT scanner has increased markedly over the last few decades, providing vets, pets and pet owners with a powerful diagnostic imaging modality. As with many established technologies, it is easy to take CT for granted and to forget that the history of CT imaging has undergone a long and fascinating journey of development to reach its current form. Understanding the history of CT imaging helps users get the most out of the technology, appreciate both its strengths and limitations, and begin to look ahead to how CT may continue to develop in the future.

Fortunately, the history of CT imaging is an intriguing one, and it begins in an unlikely place: Abbey Road Studios.

From Abbey Road to medical imaging: the history of CT imaging

Yesterday

What links Abbey Road Studios and the music of the Beatles, Edward Elgar and Kate Bush to computed tomography? Founded in 1931, the company opened Abbey Road Studios that same year, welcoming artists ranging from the Fab Four to Elton John and Amy Winehouse in the decades that followed.

As EMI’s commercial success grew, it invested heavily in research beyond music production. Its work extended into television technology, guided weapon systems and radar, among many more. It was within this research environment that Godfrey Hounsfield entered the story.

Hounsfield joined EMI in 1951 and achieved early success developing Britain’s first all transistor computers. This background in electronics and computing, combined with the financial freedom provided by Beatlemania in the 1960s, gave him the opportunity to pursue a project of his own choosing.

A problem in medical radiography

Help!

When considering what to work on, Hounsfield recalled a conversation with a doctor who had expressed frustration at the limitations of conventional radiographs for imaging the head, and particularly the brain. Traditional X-rays offered poor contrast resolution and produced a two-dimensional image of a complex three-dimensional structure. Superimposition of surrounding anatomy obscured pathology, severely limiting diagnostic value.

When pondering the problem, Hounsfield had a brainwave: what if multiple X-ray projections were acquired from different angles and combined to form a single image?

He submitted a research proposal to EMI titled “An improved form of X-ray radiography.”  In it, he described taking a series of X-ray exposures through a region of anatomy from multiple, precisely defined angles. At each angle, X-ray attenuation would be recorded and the resulting data reconstructed into a cross-sectional image, or slice, of that region.

This approach required a major advance in computer processing to handle the volume of data generated. Presenting anatomy as individual slices preserved three-dimensional structure, a technique known as tomography. When combined with computer-based reconstruction, the technology acquired its name: computed tomography.

Building the first CT scanner

We can work it out

Initially, EMI were hesitant to support the project, given their limited involvement in medical technology. However, additional funding from the British government allowed the work to proceed.

Working alongside consultant radiologist James Ambrose, Hounsfield constructed a prototype CT scanner. This early system was table based and used a narrow pencil beam of X-rays to image anatomical specimens. Rather than rotating the X-ray tube and detector around the patient, the specimen itself was rotated to obtain multiple projections.

First CT scanner prototype developed by Godfrey Hounsfield in 1968. Alex Gustschin, Source, CC-BY-SA-4.0

The concept proved successful, leading to the construction of a full-size scanner installed at Atkinson Morley Hospital in London. Compared with X-ray film, the detector was far more sensitive, delivering greatly improved contrast resolution, particularly important for imaging soft tissues such as the brain. Crucially, anatomical structure was preserved in individual slices, avoiding the problem of superimposition.

At the same time, and working independently in the United States, physicist Allan Cormack developed the mathematical theory that underpinned CT image reconstruction. In recognition of their combined contributions, Hounsfield and Cormack were awarded the 1979 Nobel Prize in Physiology or Medicine.

EMI Scanner Mark I, alongside the minicomputer used to process the CT image data, Philipcosson, Source, PDM 1.0 Universal

Early CT scanners, known as EMI Scanners, required hours to reconstruct a single image.
